BIG TOE PAIN
Stiff, aching or restricted movement in your big toe joint?
The big toe joint (first MTPJ) bears substantial weight during every step you take. Hallux rigidus, arthritis, or gout can make walking and pushing off deeply uncomfortable. Our podiatrists can assess the joint and help restore mobility

COMMON SYMPTOMS
Does any of this sound familiar?
Big toe joint pain often develops gradually or may flare acutely:
- Pain or stiffness in the big toe when pushing off while walking or running
- Reduced upward bend (dorsiflexion) in the big toe joint
- A hard, bony bump on top of the big toe joint (dorsal bone spur)
- Pain that worsens in damp or colder weather
- Sudden, excruciating, hot redness in the big toe (possible gout flare)
- Altered walking pattern (walking on the outside edge of the foot to avoid bending the toe)
Offloading the big toe joint during the push-off phase significantly reduces daily walking pain.
A clinical assessment determines whether the restriction is structural, functional, or inflammatory.

POSSIBLE CAUSES
What causes big toe pain?
Several conditions can affect the first metatarsophalangeal joint:
Hallux rigidus / Osteoarthritis
Wear and tear of joint cartilage leading to joint space narrowing and protective bone spurs.
Functional hallux limitus
The toe can bend upward when non-weight-bearing, but locks during walking due to foot mechanics.
Gout
Uric acid crystal deposition in the joint causing sudden, acute, severe pain, redness, and heat.
Sesamoiditis or turf toe
Inflammation of the tiny sesamoid bones underneath the joint or hyperextension sprains.
WHEN TO SEEK ASSESSMENT
When should you consider getting big toe pain checked?
Consider arranging an assessment if:
- The big toe joint is noticeably stiff or difficult to bend
- You are altering how you walk to avoid bending your big toe
- You notice a painful bony spur rubbing on top of your shoes
- You experience sudden, hot, severe pain in the joint
- Exercise or running causes persistent joint aching afterwards

YOUR APPOINTMENT
What happens when you come to see us?
We measure joint motion, check for bone spurs, and evaluate gait mechanics
We examine
Your podiatrist checks the range of upward bend in your big toe sitting and standing.
We evaluate
We palpate joint margins for spurs, assess sesamoid bones, and evaluate swelling.
We assess gait
Gait analysis assesses how your foot rolls off the big toe during the propulsion phase.
We plan
You receive recommendations on rocker-sole footwear, custom orthotics, or joint offloading.
TREATMENT OPTIONS
Treatment depends on what we find
Treatment aims to reduce joint irritation while preserving comfortable walking:
Not every treatment is appropriate for every patient. Your podiatrist will discuss suitable options after assessment.
- Prescription orthotics with first-ray cutouts or morton's extensions
- Biomechanical assessment & gait analysis
- Rocker-sole shoe guidance to reduce toe bend demands
- Diagnostic ultrasound imaging where indicated
- Shockwave therapy for surrounding soft-tissue inflammation
